Required by Texas Health and Safety Code §181.105 (House Bill 4224)

Your Rights as a Consumer
Texas law requires health care providers, including clinical social workers, to provide clear information about your rights related to health care records, licensing oversight, and consumer complaints.
Below you will find instructions on how to:
1. Request your health care records;
2. Contact the licensing authority that regulates this practice; and
3. File a consumer complaint with the Texas Office of the Attorney General.
1. How to Request Your Health Care Records
You have the right to request access to your health care records.
To request your records:
● Submit a written request that includes your full name, date of birth, and a description of the records you are requesting.

Requests will be processed in accordance with applicable state and federal laws, including HIPAA and the Texas Health and Safety Code. You will be notified if additional information or authorization is required.
2. Contact the Licensing Authority
This practice is regulated by the Texas Behavioral Health Executive Council (BHEC), which oversees the licensing and regulation of clinical social workers in Texas.

3. How to File a Consumer Complaint
If you have a concern related to consumer rights, access to records, billing, or other consumer protection matters, you may file a complaint with the Texas Office of the Attorney General – Consumer Protection Division.
